Q.-  We are a large employer, so we are sending our full-time employees Form 1095-C.  What will Lines 14, 15 and 16 look like if one of these employees waives coverage?

A.-  For someone who waived coverage, Line 14 will show the offer of coverage.  Assuming coverage is offered to the employee, spouse and dependent children, Line 14 will be coded 1E.  Line 15 would have the dollar amount of the lowest cost plan offered to the employee, and Line 16 would be blank. Some employers who have analyzed affordability calculations are marking one of the affordability codes (2F, 2G, 2H) on line 16 for those who waived coverage, but other employers are choosing to leave the line blank.
