MyEnroll360 Claim Submission Processes

BAS gives you the ability to make your employees happy with easy flexible spending account claim submission and timely & accurate claim processing.

BAS offers three easy ways for employees to submit FSA claims for review and reimbursement.

Online Claim Submission

Employees can submit claims online, 24x7, from their own computer.

Employees visit www.MyEnroll.com and enter their user ID and password. When they are logged in, they will be on their home page. An employee should hover his or her mouse over the word “Employees” in the top menu bar. Under “Flex Spending Accts.,” they will click on “Enter FSA Claims Online.” This will take the employee to the claim submission process where the employee will describe the claim, who incurred the claim, the date of the claim, and the employee will be able to upload the supporting claim documentation. Mailed Claim Submission

Employees can mail their FSA claims. They will need a claim form, which can be downloaded from MyEnroll.com. The employee should go to www.MyEnroll.com and select “Flexible Spending Account” from the menu bar.

  • Use the drop down menu to select, “Non-Bar Coded Claim Forms”
  • Select the format you prefer (html, PDF, or Word) and the FSA plan (Dependent Care or Health Care)
  • Print the form

The employee will complete the claim form, sign it at the bottom and gather supporting documentation. The employee should mail the claim form and documentation to

Faxed Claim Submission

BAS still accepts claim submissions by fax. To fax a claim, an employee will need a claim form, which can be downloaded from MyEnroll.com. The employee should go to www.MyEnroll.com and select “Flexible Spending Account” from the menu bar.

  • Use the drop down menu to select, “Non-Bar Coded Claim Forms”
  • Select the format you prefer (html, PDF, or Word) and the FSA plan (Dependent Care or Health Care)
  • Print the form

The employee will complete the claim form, sign it at the bottom and gather supporting documentation. The claim form and documentation should be faxed to 1.888.265.2144.
