  • What is Form 1095-C?

Form 1095-C is an IRS form for Affordable Care Act reporting. Form 1095-C provides employees with information about their health coverage and their employer’s offer of health coverage. Applicable Large Employers (those with 50 or more full-time employees in the prior year) must furnish Form 1095-C to their full-time employees confirming the offer of health coverage to employees and their dependents.

  • What is Form 1095-B?

Form 1095-B is an IRS form for Affordable Care Act reporting. Form 1095-B provides employees with information about their health coverage. A health insurance company or issuer/coverage provider furnishes Form 1095-B to covered individuals. Form 1095-B is also furnished by a small employer (one with fewer than 50 employees) that has a self-funded health plan. The form identifies the employees’ coverage along with any dependents’ coverage.

  • What are Forms 1094-B & 1094-C?

Form 1094-B and Form 1094-C are IRS forms used to transmit health plan information to the IRS. The 1094 transmission provides the IRS with summary information about coverage and offers of health coverage and includes copies of all 1095 forms furnished to employees. Each employer (location) with an EIN or entity tax ID number recognized by the IRS makes its own 1094 transmission to the IRS.
