Three Ways to Obtain an FSA Claim Form

BAS often receives requests for copies of Flexible Spending Account (FSA) Claim Forms. Here we highlight three ways to quickly access a blank FSA claim form.

1. MyEnroll logon not required: Go to www.myenroll.com. Select “Flexible Spending Account” from the top menu bar. Use the drop down menu to select, “Non-Bar Coded Claim Forms.” Select the format you prefer (html, PDF, or Word) and the FSA plan (Dependent Care or Health Care). Print the form.

2. Bar Coded, Personalized Form: Go to www.myenroll.com. Select “Flexible Spending Account” from the top menu bar. Use the drop down menu to select, “Bar Coded Claim Forms.” Scroll down and click on the link titled, “Click Here to Open Your Personalize, Bar Coded Claim Form!” Enter your MyEnroll user ID and password, when prompted. Choose “Generate” next to the Health Care FSA or Dependent Care FSA. Select the appropriate dependent. The appropriate pre-populated form will generate. You can enter your claims, then print, sign and submit with your receipts. Upon receipt of your claim, MyEnroll will send you an email notice that your FSA claim is being processed.

3. Already Logged in to MyEnroll: If you are already logged in to MyEnroll, you can quickly access your Bar Coded FSA Claim Form. Using the left menu, select, “Bar Coded FSA Forms.” Then, choose the FSA plan (Dependent Care or Health Care) and the appropriate dependent. The appropriate pre-populated form will generate. You can enter your claims, then print, sign and submit with your receipts. Upon receipt of your claim, MyEnroll will send you an email notice that your FSA claim is being processed.
