Q.- We are preparing our 1095-C Forms. We don’t have the Social Security Number for some employees’ covered dependents. The Form asks for SSN. What should we do?


A.- If you were not able to obtain Social Security Number, use the dependent’s birth date in Part III of the form. You should have records that you requested the Social Security Number but were not able to get it.
