HIPAA Breach After Excel Sorting Goes Wrong

A major southern university medical center had to send breach notification letters after an Excel spreadsheet sort went wrong. 

The University was involved in a Medicaid wellness initiative.  The University contacted physicians to obtain certain information from patient records.  Letters were sent to the wrong physicians after patient and corresponding physician data was sorted incorrectly.

About 400 people were impacted.
