2015 Draft Health Care Reform Reporting Forms Released

The IRS released draft versions of 2015 Forms 1095 and 1094. These forms, once finalized, will be used to report information to employees and to the IRS under health care reform. 

The draft 2015 forms are almost identical to the 2014 versions.  Instructions for the 2015 forms were not yet released. Changes include 

Form 1095-C

  • the addition of a new field- "plan start month." It is an optional field and will be used to indicate the first month of the large employer’s plan year.
  • the addition of a continuation sheet if more than 6 dependents have to be reported
  • a new indicator code to be used for 2016 (not 2015) 

Form 1094-C

  • unchanged 

Form 1094-B

  • the addition of a continuation sheet if more than 6 dependents have to be reported
