Easy Access to FSA Claim Forms

BAS offers tools to make the Flexible Spending Account reimbursement process easy for employees.

FSA claim receipt and documentation must be accompanied by a completed and signed claim form. Participants may obtain a claim form in one of three ways:

1. MyEnroll logon not required: Go to www.MyEnroll.com. Select “Flexible Spending Account” from the top menu bar. Use the drop down menu to select, “Non-Bar Coded Claim Forms.” Select the format you prefer (html, PDF, or Word) and the FSA plan (Dependent Care or Health Care). Print the form, complete it and submit with documentation.

2. Bar Coded, Personalized Form: Go to www.MyEnroll.com. Select “Flexible Spending Account” from the top menu bar. Use the drop down menu to select, “Bar Coded Claim Forms.” Scroll down and click on the link titled, “Click Here to Open Your Personalize, Bar Coded Claim Form!” Enter your MyEnroll user ID and password, when prompted. Choose “Generate” next to the Health Care FSA or Dependent Care FSA. Select the appropriate dependent. The appropriate pre-populated form will generate. Print the form and submit it with documentation.

3. Already Logged in to MyEnroll: If you are already logged in to MyEnroll, you can quickly access your Bar Coded FSA Claim Form. Using the left menu, select, “Bar Coded FSA Forms.” Then, choose the FSA plan (Dependent Care or Health Care) and the appropriate dependent. The appropriate pre-populated form will generate. Print the form and submit it with documentation.
