Employees who do not want to submit online requests for reimbursements from their flexible spending account plans or health reimbursement plans can send receipts through the US mail, by fax or by email. Each reimbursement submission must be accompanied by a signed and dated claim form.
Printing a claim form is easy and employees do not even have to log in to get a form. On the home page of www.myenroll.com, employees can click on the “Forms” button at the top. From the drop down menu, select the form to print.
